APCM services combine elements of several existing care management and communication technology-based services you may have already been billing for your patients. This payment bundle reflects the essential elements of advanced primary care, including:

Principal care management (PCM) – disease-specific services to help manage a patient’s care for a single, complex chronic condition that puts them at risk of hospitalization, physical or cognitive decline, or death
Transitional care management (TCM)
Chronic care management (CCM)

Communication technology-based services include:

Virtual check-ins
Remote evaluations of pre-recorded patient information
Interprofessional consultations

APCM services allow you to:

Provide patients with a wide range of services to meet their individual needs based on complexity
Bill for these services using a monthly bundle (instead of billing for each individual service or recording minute by minute)

These services help simplify your billing and documentation requirements while ensuring that your patients have access to high-quality primary care services.

WHAT WE WILL DO FOR YOUR PRACTICE

PROVIDE 24 HOUR / 365 DAYS OF ACCESS TO OUR CARE TEAM.
EDUCATE AND ENROLL YOUR PATIENTS IN THE ADVANCED PRIMARY CARE MANAGEMENT PROGRAM.
PROVIDE A DEDICATED CARE TEAM TO ENGAGE YOUR PATIENTS ON A MONTHLY BASIS.
WORK WITHIN YOUR EMR FOR IMMEDIATE ACCESS TO NOTES AND CARE PLANS.
MAINTAIN COMPLIANT AND DETATILED NTOES OF ALL INTERACTIONS.
PROVIDE A ECOSYSTEM OF COMMUNITATION BETWEEN THE PRACTICE, YOUR PATIENT AND ANY ADDITIONAL CARE AGENTS (PHARMACIES, SPECIALIST, CARETAKERS).
MONTHLY ENGAGEMENTS WITH YOUR PATIENT VIA PHONE, TEXT AND/OR EMAIL BASED ON NEED.
INCREASED PATIENT CENSUS
INCREASED ANNUAL WELLNESS VISITS (AWV)
INTRODUCE AN ADDITIONAL 100K OF REVENUE TO YOUR PRACTICE

INFORMATION FOR YOUR PATIENTS

Medicare Part B (Medical Insurance) now pays for Advanced Primary Care Management services to help your doctor and other health care providers coordinate and tailor care to your needs each month.

YOUR COST IN ORIGINAL MEDICARE

After you meet the Part B deductible, you pay 20% of the Medicare-approved amount. Many people who have both Medicare and Medicaid won’t pay anything for these services. If you have a Medicare Advantage Plan, your costs for these services might be different. Contact your plan for specific cost information.

WHAT IT IS

Depending on the primary care services you're currently getting, you may get the same or more services through Advanced Primary Care Management. Providers that offer these services must provide you with:

24/7 access to your care team or provider through the phone, an online portal, email, and virtual check-ins
Chronic care management
A personalized care plan that lists your current conditions and treatments
Comprehensive care coordination to quickly connect you with specialists and other providers
Transitional care between health care settings (like follow-up are after you're discharged from a hospital)
Medication management

THINGS TO KNOW

Check with your primary care provider to make sure they offer these services. Then, give your provider verbal or written consent to be your main point of care for all your health needs. They’ll document your consent on your medical record. You also need to have an initial visit with your provider unless:
- You've gone to the provider or another provider in the same practice within the last 3 years.
- You got another type of care management (like chronic care management) from the provider or another provider in the same practice within the last year.
You can stop getting services at any time. Talk to your provider to stop participating.
